Community Based Day Services
STREAM stands for Success Through Relationships, Employment, And Meaningful experiences. STREAM is a person-centered program that offers a combination of work, training, volunteerism, enrichment, and recreation based on each person’s individual goals and needs.
At intake, the individual’s current case managers will meet with each STREAM candidate and their support network to identify these goals. STREAM staff will then assist the individual in working towards their goal by providing individualized support for each client and the activities the individual chose for the day.
Upon enrollment, the individual will begin in the STREAM program and will begin choosing activities for their free time from a regularly updated activity calendar. Activities within the classroom include math, science, art, workplace learning, reading, writing, etc. Our activities that occur within the community include sites to volunteer locations, museums, libraries, shopping, bowling, lunch club, etc. within the surrounding towns of our sites in Gardner and Orange. Individuals are transported to site-based activities by utilizing GAAMHA’s fleet of vehicles and accompanied at all times by trained staff.
Individuals experience their best daily lives through a supported self-discovery process, person-centered plans can change at any time to allow for new paths and different opportunities. Students preparing to transition from the school system to community programs also have the opportunity to participate in our STREAM program. Contact us to find out more.
